The Patient Centered Medical Home practice has integrated oral and behavioral health care along with many family centered case management programs. Volunteer: Behavioral Health Nurse Care ManagerSCOPE OF ASSESSMENT AND PATIENT MANAGEMENT: The Nurse Care Manager will have the opportunity to work on a multidisciplinary healthcare team in a community health clinic care setting. The Nurse Care Manager will be part of a program charged with working within the patient care medical home and administering health care in a new and innovative way.
